Nurses (LPN/RN) for Care Management Responsibilities

We have several excellent opportunities for LPN's and RN's to serve as Geriatric Care Managers. These are newly created, immediate opportunities that offer market-competitive pay, flexibility of schedule and the chance to work in a case management capacity.

Our LPN's/RN's provide care management services to a (primarily) geriatric patient population in a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client's health needs and to reduce hospital readmissions and emergency room utilization.

Working in collaboration with medical providers, hospital discharge planners and patient families our LPN's/RN's are assigned caseloads that are 100% per diem and can vary from short-term (3 visits) to longer term assignments.

Our LPN's/RN's monitor and document all related notes and activities via a user-friendly, proprietary software program on an ongoing, always current basis (full training provided).

Specific Requirements

  • RN's applying must have a degree of no less than one year old and some geriatric and/or case management experience
  • LPN's applying must have at least two years experience in geriatric and case management experience 
  • Licensed Social Workers (LSW) applying must have a license no less than one year old and have at least one year of geriatric experience 
  • Bachelors in Social Work (BSW) applicants must have at least two years of geriatric experience 
  • Masters in Social Work (MSW) applicants must have at least one year of geriatric experience
  • Must be comfortable with electronic documentation procedures and willing/available to complete related training
  • Additional Information

    Need per diem RN's in Winslow, Payson Williams, Congress, Seligman and Ashfork
