As a(n) Case Manager (Non-Exempt) you will: 

Assists program participants in developing vocational goals reflective of their skills, capabilities and interests. Manages participant progress towards achieving goals by creating and updating plans.

  • Meets with participants to identify barriers to employment and develop strategies to manage these issues.
  • Maintains complete, accurate and timely case notes and related records in manual and or electronic systems.
  • Outreach to referrals who fail to show for appointment.
  • Create update and ensure completion of plans according to contract deadlines.
  • Attends scheduled supervision meetings to discuss participants progress.
  • Other duties as assigned

Position Requirements

  • Bachelor’s degree in related field or a satisfactory equivalent of education and experience
  • Strong communication skills
  • Knowledge about community resources.
  • Able to work with and relate to participants and be culturally sensitive.
  • Strong organizational skills.
  • Must be computer literate and have MS Word and Excel skills at minimum, and the ability to learn specialized databases and software systems
  • Bilingual a plus

Additional Information

The Reentry Case Manager is a Grant funded position with two years of service deliverables and one year of follow-up services. Local travel required. Preferable a Bachelor’s degree in criminal justice or social work. Preference for experience working with re-entry programs, social justice, legal advocacy, and/or Fair Chance Hiring. The Reentry Case Manager will perform the following job duties. • Authorize and coordinate training services and supportive services that may include , transportation, work uniforms, equipment ,etc., and track obligations of funds; • Responsible for establishing professional relationships with employers and community agencies with a focus on developing work experience sites for individuals that experience barriers due to a criminal background; • Conducts orientation for participants to explain the benefits of the work experience program; • Interviews participants to gather information on correct appropriate site match and make timely referrals to work experience locations; • Tracks work experience referrals and number of participants at each location on a monthly basis; • Work with potential work experience sites to complete cooperative agreements; • Reviews timesheets and work readiness tool for accuracy; • Submits monthly report of program activities; • Follow-up with employees and employers to ensure expectations are met.
