But we are also connecting the NeuroCare continuum, providing expertise in the right place at the right time with outpatient and residential services in addition to our groundbreaking home and community rehab.

As a(n) Case Manager - Residential Youth Services you will: 

  • Assessment of each child’s needs, strengths and weaknesses.
  • Schedule and Develop individual treatment plans.
  • Linkage or referral of individual to resources and services both within the agency and among the inter agency network
  • Monitoring the implementation of an individual’s treatment plan to assure services are being provided as planned.
  • Advocacy on behalf, of the individual to assure accessibility to services.
  • Schedule and convene monthly Treatment Plans and Treatment plan updates.
  • Assure the availability of necessary educational services, request school grades, make regular contact face to face at student’s school.
  • Participate in on-call crisis rotation.
  • Document all case management activities conducted on behalf of the individual.
  • Provide behavioral support plan, development, review and modifications as directed.
  • Attend judicial review, custody reviews, and court hearings on behalf of the individual.
  • Work closely with the referring entity to ensure proper case coordination.
  • Complete monthly summaries to referring entity
  • Work cooperatively using a team approach.
  • Help children adjust to the transitions involved in separation and placement, and evaluate the impact of services provided for the child.

  • Conviction about he capacity of people to grow and change.

  • Ability to set limits and maintain the helping role and to intervene appropriately to meet the needs of the persons served or other family members.
  • An ability to understand and assist with the role changes that occur in foster and kinship care.
  • A commitment to a child's right to belong to a family.
  • A commitment to preserving, whenever possible, a child's connection to the family of origin.
  • A commitment to reunifying a child with his/her biological family, whenever possible.
  • BSW or Bachelor’s degree in a human services field. 
  • 1 Year experience in child welfare field.
  • Must have valid driver’s license.
  • Must pass all state required background checks.
  • Minimal Travel
  • LSW, Provisional or Temporary Social Work License, CPR, CPI and Fist Aid
