Case Manager (Wraparound Services Program) Responsibilities

The Wraparound Services Case Manager works with our eligible Welfare-to-Work (WTW) participants to provide individualized case management that is beyond the normal services provided by the CalWORKs/WTW program to help participants overcome barriers or challenges that affect their employment readiness and self-sufficiency. The Case Manager will be a part of a multi-disciplinary team to assess and remove barriers to self-sufficiency for their assigned case load.  The incumbent will provide quality case management in accordance with the CalWORKs Program Guide (CPG).

Essential Job Responsibilities:

  • The Wraparound Services Case Manager will provide career guidance and counseling following the completion of a thorough individual assessment.  
  • Collaboratively develops an individualized plan of care, implements this plan and evaluates its success over time  
  • Provide intensive case management with a focus on assisting participants to overcome barriers and prepare them for employment that leads to self-sufficiency
  • Will require participant home visits or meet with participants at outside venues to provide case management services as needed
  • Maintain case management files ensuring confidentially; ensuring files are maintained as required by the company and contract
  • Develop and maintain ongoing resources and professional relationships within the community
  • Refer participants to appropriate agencies for specialized services and/or counseling
  • Provide and documents individual/group counseling and referral services
  • Utilize county and company computer data processing systems to efficiently track and record participant activity 
  • Complete all necessary case management documentation and forms; maintain records and files; and schedules appointments 
  • Determine the need for supportive services and issue additional funds and necessities to participants as needed
  • Other duties as assigned

Specific Requirements

  • MSW Degree or current LCSW is required
  • At least 2-years of work experience as a Case Manager in social services, non-profit or for-profit organization
  • Working knowledge of the CalWORKs program and Welfare-to-Work component 
  • Ability to organize, prioritize, and complete a large volume of work within strict time deadlines 
  • Outstanding communication skills, both written and oral; must communicate professionally and positively with participants, staff, management, program partners, agencies, employers, landlords, mechanics, hotel/motel operators, and other service providers 
  • A fundamental understanding of computer processing including word processing and spreadsheet applications 
  • Maintain and establish effective working relationships with coworkers, agencies, employers, and other service providers 
  • Ability to exercise appropriate judgment in releasing HIPAA/PHI information; analyze decisions and/or recommendations relating to the participant delivery of services
  • Requires maintaining confidentiality of files and records that are considered personal and sensitive 
  • Ability to work with a diverse population and apply strong observation, analytical and listening skills

Work Requirements:

  • Must maintain LCSW and renew every 2-years as required by the State of California
  • Proficient office computer skills; MS Word program applications  
  • Current Driver’s License/ Drivers Insurance and have and maintain a “Good Driving Record” 
  • Must have access to an insured currently registered car daily and during regular work schedule
