Health and Safety Case Manager - Seattle ResponsibilitiesAssists eligible recipients to gain access to needed medical, behavioral, health, social, educational and other services. Establish processes to coordinate a range of services for instruction and assistance for persons served and their families.

Specific Requirements

  • Bachelor’s degree in a human services field. 
  • Valid driver’s license.
  • Must have two years of full-time, post-bachelor’s degree accumulated MH/DD/SA experience with the population served.
  • Must be able to communicate both verbally and in writing.
  • Must be able to bend, stoop, push, pull, reach, sit and walk for periods of time.
  • Must be able to utilize proper body mechanics while assisting with transfers of individuals weighing up to 250 pounds.

Additional Information

The Health and Safety Case Manager is responsible for over-all health and safety supervision of each program site. Ensures that policies and procedures are being followed, minimum standards for Status and Dignity are being met, Residential Guidelines are being implemented, WAC and Division of Development Disabilities documentation is maintained. Duties and Responsibilities Include (but are not limited to):- Medication Documentation- Nurse Delegation- Completing and Monitoring Psychoactive Medication Plans- Tracking Dietary Needs- Ensuring Medical Appointments are Completed- Training First Aid/CPR- Conducting Site Visits- Assist in Finding and Facilitating Services as Needed- Ensure that Emergency Preparedness Plans are Present and Up-To-Date
