Licensed Professional Counselor - Rehab Without Walls - Outpatient Center Responsibilities

Provides psychological treatment, assessment, and wellness promotion to rehabilitation patients and their families. Assists the individual and his/her family in achieving optimal psychological, interpersonal, and physical functioning. Confers with the treatment team regarding the impact of brain-behavior relationships on the person’s served daily functioning. Provides psycho-education to the treatment team members, the person served, and his/her family regarding the implication of psychological deficits in daily functioning. Works under general supervision.


Essential Job Responsibilities

  • Abides by and demonstrates the company’s Mission, Vision, and Values through both behavior and job performance on a day-to-day basis.
  • Provides empirically based psychological interventions to the person receiving services and his/her family as appropriate for their specific needs in order to maximize and encourage their participation with the community at large. Interventions may include evidence-based counseling and psychotherapy, behavior management, cognitive remediation assertiveness training, problem solving focused interventions, addressing adjustment processes and coping, psycho-education, stress management, wellness promotion, compensatory strategies, pain management, as-well-as programmatic outcomes.
  • Performs brief neurobehavioral assessments at various stages in the rehabilitation process as appropriate.
  • Provides feedback to the person served, family, and treatment team, with regards to behavioral, emotional, psychological, and cognitive rehabilitation issues, test results, and practical applications.
  • Designs community reintegration activities to assist the person receiving services with their psychological/cognitive needs during physical activity.
  • Assists the person receiving services in understanding their needs and limitations, and how to manage them during function.
  • Performs a thorough evaluation by assessing the needs of the person receiving services, and sets measurable objectives that are formulated in conjunction with the rehabilitation team.
  • Adjusts treatment to achieve maximum results.
  • Responsible for applying professional knowledge and skills in accordance with the accepted standards of clinical practice, to facilitate problem resolutions, and thereby the achievement of individualized goals and outcomes for the person served.
  • Provides treatment progress and status documentation in accordance with the Plan of Treatment.
  • Maintains and submits documentation in the format and timelines required by the Company, facility, local, state and federal regulations.
  • Reports progress to the rehabilitation staff, person receiving services and family members.
  • Observes/documents and reports changes in the condition of the person served to the clinical coordinator/designee, attending physician, and other treatment team members as appropriate.
  • Participates in the team process, provides input and effective treatment strategies.
  • Attends case conference and family educational meetings.
  • Develops, teaches, and assesses the Plan of Treatment.
  • Uses appropriate methods and tools in all teaching and training activities, including those associated with discharge.
  • Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Shares information only when the recipient’s right to access is clearly established and the sharing of such information is clearly in the best interest of the person served.
  • Adheres to and participates in Company’s mandatory HIPAA privacy program / practices and Business Ethics and Compliance programs / practices.
  • Collects and documents items necessary for the organization to complete thorough and effective program evaluation and program improvement.
  • Attends, participates in, and/or conducts internal staff development programs, obtains continuing education as required by company policy and regulations.
  • Maintains and enhances clinical practice skills.
  • Maintains a professional appearance, is reliable and punctual, cooperative and conducts self according to accepted standards of professional practice in the course of assigned duties.
  • Performs other duties as assigned in response to the person served, office, and/or program needs.
  • Reviews and adheres to all company policies and procedures.
  • Participates in special projects and performs other duties as assigned

Specific Requirements

  • Master’s or doctoral degree in counseling or a counseling-related field from an accredited college or university
  • A minimum of one year of clinical experience and counseling individuals with traumatic or acquired brain injuries or related neurological conditions.
  • Knowledge of complex neurologic injury rehabilitation techniques.
  • Maintenance of professional license and/or certification as required by the state in which the counselor practices
  • Demonstrates effective and professional interpersonal, verbal, and written communication skills.
  • Able to work independently and part of a team.
  • Organized and able to function under minimal supervision.
  • Successfully completes any/all required pre-employment evaluation test(s) per policy in each assigned area of practice.
