Essential Job Responsibilities:  
  • Compile, process, and maintain medical records, of person(s) served in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health services system.
  • Report patient information for health requirements and standards.
  • Assist Director of Health Services and Health Services Supervisor (or area designees) with documentation.
  • Maintain closed medical records according to are regulatory requirements.
  • File laboratory slips, x-ray documents, etc. to current chart.
  • Phone physician for appointments for individuals.
  • Purge and file programming documents, re-supply forms and sheets each month.
  • Provide medical record information to other operations, agencies and physicians’ offices.
  • Supply nursing personnel with forms and documents required.
  • May type discharge summaries and may prepare quarterly letters to person(s) served.
  • Provides requested information to Resource Center staff as requested by staff other than nursing.
  • May type letters to guardian/advocate to obtain signatures for Service Plans or Team reviews.
  • Maintains skills acquisition and data collection files.
  • Maintains files relating to person(s) served financial information.
  • Completes routine audits of information files.
  • Makes copies of Medicaid cards and distributes to locations by the first of each month.
  • Assures document copies have been provided to each location and person served as needed.
  • May assist with writing objective/criteria sheets.
  • Assures charts and/or files are purged on a regular basis, monthly and/or as required.
  • Maintains all original information files for each person served.
  • Maintain up-to-date face sheet for each person served and input necessary information into database, if applicable.
  • Maintains soft folder for overflow papers and forms for each person served.
  • Other duties as assigned.

Job Requirements


High school diploma or GED equivalent.

Certificates, Licenses, Registrations:
Valid driver’s license from state of residence.

This is a part time office position, M-F with a negotiable schedule covering 20 hours a week.
