RN for Case Management Responsibilities

  • Responsible for tracking labs for new admits and current patients. Knowing when the labs are due and verifying completed. Faxing all results to MD.
  • Receiving all reported PT/INR results from clinicians and faxing to MD. Follow through with clinicians to verify orders were received from MD and turned into CSS.
  • Pink PUF D/C forms reviewed and given to oasis for more timely processing or returned to clinician if inappropriate discharge completed.
  • Cross Train-Intake Coordinator, CSS, Insurance Specialist and Clinician Manager.

    • Specific Requirements

  • Graduate of an accredited school/college of Practical Nursing as a Licensed Practical Nurse with current registration in state of primary work site location.
  • Some experience as an LPN in home healthcare preferred but not required.

    • Additional Information

    Our PRN nurses provide case management and assess patient health problems or needs in the home of the patient. Advise patients on health maintenance and disease prevention. Provide health teaching of client and family, and assisting client in learning appropriate self-care techniques. Assign or arrange for the services of other disciplines for a client, such as PT, OT, a Speech Pathologist, and MSW. Maintain medical records in data base.
