About this Line of Business

The Wellness, Comprehensive Assessment, Rehabilitation and Employment (WeCARE) Program provides a continuum of integrated services that evaluate the abilities and address the needs of Cash Assistance (CA) clients with medical and/or mental health condition(s) that may affect their employability. Clinical assessment processes determine clients’ functional capacity outcomes and ability to achieve the goals of wellness and employment or to apply for and receive federal disability benefits.

The WeCARE program has been successful in assisting thousands of New York City clients with clinical barriers achieve self-sufficiency. ResCare Workforce Services is a funded provider for WeCARE services through the New York City Human Resources Administration (HRA).

As a(n) VRS Fast Track Care Manager Lead (Placement) you will: 

  • Leads a team that is responsible for placing work ready customers into employment and ensuring they retain employment
  • Ensures customers are supported to help them carry out a successful job search, gain employment, and be retained in employment for one year
  • Oversees the conduct of adequate workshops for employed customers to support and reinforce job retention skills
  • Trains staff to complete Individual Vocational Assessment and Plan (IVAP) documents with customers, incorporating the customer’s input to their vocational and employment goals and the steps to achieve goals. Ensures these plans are in place for all customers. 
  • Supports assigned staff to use clinical assessment documents as necessary to support the IVAP development process
  • Manages staff who provide case management, coaching, and counseling related to customer pursuit of IVAP activities and goals
  • Ensures the team is conducting outreach to support customers remaining engaged with IVAP activities
  • Coordinates with other providers of services for customers to ensure the team is able to connect customers to the support needed to find and retain employment
  • Receives and uses daily, real-time feedback on the effectiveness of their work and uses it to improve performance as warranted
  • Understands and follows all HRA policies and procedures related to vocational rehabilitation services
  • Uses standard forms and online processes to conduct and document work activities

Position Requirements

  • Bachelor’s Degree or higher in social services or a related field such a psychology, vocational rehabilitation, or occupational rehabilitation
  • Four years direct customer service experience providing case management in a clinical or social service environment plus two years’ of supervisory experience
  • Experience in a fast-paced, team environment that is performance driven
  • Exceptional customer service and engagement skills; prefer training or experience in motivational interviewing and care management techniques
  • Excellent organizational skills and strong attention to detail
  • Experience with publicly-funded programs; TANF experience preferable
  • Experience in assessing the need for and making reasonable accommodations for customers 
  • Knowledge of laws and regulatory requirements related to program responsibilities 
  • Ability to use various electronic systems to carry out duties and responsibilities
  • Bilingual or multilingual preferred
